One of my responsibilities while working as the HIV Prevention Program Manager at the Kansas Department of Health & Environment was to develop reports and documents requested by the Centers for Disease Control and Prevention (CDC) as part of the grant requirements for funding we received from the federal government. This is an example of one of the plans they required in connection with our funding in 2012.

The Comprehensive Plan describes planned activities, objectives, and goals for HIV prevention services across the State of Kansas for the years 2012 to 2016. CDC provided a template in MicroSoft Word for the report format.
